Case Manager
Location: Springfield, MO
Department/Program: Support Services for Veterans (SSVF) Program
Primary Shift: M-F, 8:00 am – 5:00 pm
Job Classification: Full
-Time
FLSA Classification: Non-Exempt
Salary: $16 – $17 per hour
Travel Required (Y/N, %): Y, 50%

What you’ll do:
Develop a relationship with clients and their families to provide advocacy and direction to ensure the client has the best means to reach self-sufficiency.
Provide housing stability case management services for homelessness and at-risk homeless individuals and families.
Advocate and interact with other service providers, churches, and community organizations on behalf of participants and facilitate referrals.
Provide extensive case management, including minimal bi-weekly contact with each client in combination with phone and face-to-face visits in varied settings, including the client’s home.
Complete written and data-based documentation (ie., internal documentation case notes, housing plans, etc.)
Attend community meetings as determined by the supervisor.
Perform other related duties as assigned.

Possesses a strong commitment to the mission of Catholic Charities, including the understanding of Catholic Social Teaching and the ability to demonstrate the philosophy and values of the agency. All you need for success:
Minimum Qualifications
Required:
Bachelor’s degree in social work (BSW) or equivalent human services degree.
This position requires a valid driver’s license, a reliable vehicle, and proof of insurance.
Preferred:
Experience in case management and homeless/at-risk of homelessness populations.
Knowledge of social services and referral sources for families.
Motivational interviewing skills.
