Role Description The Case Manager is a full-time, on-site role based in Aurora, IL, responsible for supporting individuals and families in accessing services and achieving their personal, educational, and economic goals. Day-to-day responsibilities include conducting intake assessments, developing individualized service plans, and coordinating referrals to community resources such as housing assistance, workforce training, and educational programs. The Case Manager monitors participant progress, provides ongoing coaching and advocacy, and maintains accurate, timely documentation in accordance with organizational and funder requirements. The role also involves collaborating with partner agencies, participating in community outreach events, and contributing to program development and evaluation to improve service delivery.
Qualifications

  • Direct service and case management skills, including intake interviewing, needs assessment, service planning, and follow-up.
  • Community resource navigation skills, including knowledge of local social services, housing programs, workforce development, and educational support systems.
  • Communication and interpersonal skills, including active listening, clear verbal and written communication, and the ability to build trust with diverse participants.
  • Organizational and documentation skills, including accurate recordkeeping, time management, and use of case management or client tracking software.
  • Cultural competence and advocacy skills, including working effectively with underserved communities and addressing barriers to equity and opportunity.
  • Ability to collaborate with multidisciplinary teams, partner agencies, and volunteers to coordinate comprehensive support for participants.
  • Bachelor’s degree in social work, human services, psychology, counseling, or a related field; equivalent experience in case management or community-based services may be considered.
  • Experience in nonprofit, community-based, or social service settings, preferably serving low-income or displaced residents.
  • Strong problem-solving skills and the ability to respond calmly and effectively in challenging or crisis situations.
  • Proficiency with standard office software (e.g., Microsoft Office, email, databases) and willingness to learn new systems.
