Reports to: Director of C4
Hours: Full-time
Salary: $53,700
FLSA: Non-Exempt

Position Summary:
The Community Child Care Connection (C4) Case Manager will assist in engaging parents with children from birth up to thirteen (13) years old, with an emphasis on children that are five (5) years old and younger. This role works closely with NYC Benefits team to complete application and enrollment process and ensure that individuals are eligible and aware of all options. The program will ensure all eligible East Harlem residents are aware of the support available to them and how to access, enroll and stay enrolled in those support programs.

Responsibilities

Assist in the expansion of existing benefits across capacity to reach a larger number of East Harlem residents that are already well-served by the organization and/or connect clients to additional benefits.
Conduct outreach and facilitate information sessions to inform community of current C4 Programming.
Track and monitor the program’s implementation, including tracking information for clients, enrollment, benefits types, vouchers and follow-ups.
Maintain documentation of services for tracking and reporting purposes to ensure compliance with contractual requirements.
Attend all meetings with partners, and other city and community providers to enhance the program’s capacity to enroll all eligible residents into available benefits and services.
Build and maintain partnerships and collaborations with other East Harlem service providers to enhance the program’s capacity to meet the needs of East Harlem residents for services and benefits.
Perform other duties as requested by supervisor or management.

Qualifications:

High School diploma or higher.
Must have at least 3 years of experience in an educational, community, or business setting serving families, children, or adults.
Multilingual English/Spanish, English/Chinese
Willing to travel locally to promote program and events.
Strong interpersonal communication skills.
Strong writing, analytic, technical, and organizational skills
Knowledge of principles of family support and case management.
Knowledge of East Harlem community strongly preferred.
