Position Summary:
The Case Coordinator plays a vital role in supporting children with cancer and their families by coordinating services, administering direct assistance programs, and acting as a compassionate point of contact. Working under the guidance of the Program Director, this position helps ensure timely and efficient delivery of resources while maintaining clear communication and accurate documentation.
Key Responsibilities:
Serve as the primary contact for assigned families, offering ongoing support, empathy, and clear communication throughout their cancer journey.
Process and track applications for financial assistance programs and ensure eligibility documentation is complete and up to date.
Coordinate services and referrals between ACFC programs and external community partners.
Maintain organized, accurate case notes and client records in accordance with foundation guidelines and privacy standards.
Collaborate with the Program Director to assess family needs and identify opportunities for additional support.
Participate in regular team meetings, case reviews, and staff trainings.
Assist with data collection and reporting for program evaluation and grant purposes.
Support events and outreach efforts as needed to maintain strong connections with the families we serve.
Qualifications:
Preferred Bachelor’s degree in Social Work, Child Life, Public Health, or related field preferred.
Minimum of 2 years of experience in case coordination, social services, or a nonprofit setting.
Bilingual (English/Spanish) strongly preferred.
Strong interpersonal and communication skills with a high level of empathy and cultural sensitivity.
Detail-oriented with excellent time management and organizational skills.
Familiarity with pediatric illness, trauma-informed care, or medical social work is a plus.
Proficiency in Microsoft Office and experience with client management systems or databases.
