Founded in 1826 as a home for orphaned children, St. John’s has evolved into a nonprofit social services agency providing residential care to New York City youth involved in the child welfare and juvenile justice systems. We offer holistic, trauma-informed support to young people up to age 21.
Our main campus in Rockaway Park, Queens, houses a Residential Treatment Center, Close to Home Non-Secure Placement programs, administrative offices, clinical services, and an on-site NYC Department of Education school. We also operate a Non-Secure Detention residence in Richmond Hill and Limited Secure Placement programs in Brooklyn and Staten Island.

Summary Job Description:
Works in partnership with social service and residential departments as an integral member of the treatment team to provide direct services to youth and their families. Must follow programs Evidence Based Model in developing and executing a permanency plan for youth. Must be creative, have excellent verbal and time management skills and ability to develop trusting relationships with youth and their families. Works intensively with team in developing and executing goals to assure a safe and sound release plan. Provides crisis intervention, task oriented and trauma informed services to youth and their families. Develops visitation plans for families and sets up monthly treatment team schedules.
Principal Responsibilities:
- Demonstrates fidelity to the agency’s service values and organizational imperatives.

Schedules and assists with coordination of parent/child and/or sibling visitation schedules and ensures follow through/ compliance. Submits to team weekly.
- Coordinates with Case Planner and develops monthly treatment plan conferences.

Transport and attends court proceeding, medical, clinical, educational, vocational, housing and etc. appointments on behalf of youth, with youth and program staff; advocates for youth as appropriate.
- Attends and actively participates in treatment team meetings, release/discharge conferences, Red Flag meetings, Family Team Conferences, Transitional Meetings, IEP conferences and other meetings related to the resident plan.
- Meets with youth, as needed to discuss safety, well-being, permanency, and transitional progress, provide crisis intervention and task centered treatment.
- Enters progress notes inclusive of all face- to- face contact/collateral and relevant case information regarding youth into Connections database.
- Meets regularly with supervisor for supervision to discuss case/task related challenges, progress and ideas.
- Attends scheduled unit meetings with Director of Social Service, caseworker and other support staff, to discuss challenging cases, exchange treatment ideas and receive updates regarding agency, ACS and OCFS policies, for compliance.
- Administrative duties to include but not limited to input and tracking of incident reports, visitation, resident placement disruptions (MYEVOLVE), resident restraints (ESPIS) into ARTS system (OCFS system)/ Connections, and AWOC and critical incident documentation.
- Attends school conferences and IEP meetings. Observes youth’s behavior and academic progress during weekly school visits. Meets with youth in respective program/unit to discuss observations, reports and offers support, guidance and suggestions.
- Engages with parents during home assessments, treatment team and release/discharge conferences. Assist in obtaining medical consents and vital documents parents. Provides support to parents through excellent listening skills. Provides technical support to parents as needed.
- Attends required/ offered training sessions at ACS, OCFS and agency.

Provides temporary coverage of uncovered cases as needed
- All other duties as assigned.
Minimum Requirements:
Bachelor’s degree in Human Services or related field preferred -OR- an Associate’s degree Human Services related field; A valid NYS Driver’s license in good standing. Knowledge of child welfare system preferred, specifically in congregate care setting. The ideal candidate is professional with a strong work ethic; Ability to multi-task with a focus on prioritizing; Strong organizational, presentation and writing skills; Works until the job is complete; Able to think/ work independently, manage crisis/ conflict; Works effectively as a member of the team; Able to effectively engage resident youth, despite possible behavioral/cognitive challenges; Fidelity to Missouri Approach/ program specific therapeutic intervention.
