OKCIC is hiring a RN Care Manager! This position will provide professional nursing care and collaborate with other healthcare professionals in planning, organizing, and coordinating the care and treatment of patients. The care manager provides medical education counseling and manages the nursing and clinical duties of assigned teams, involved in clinical projects. This position will provide nursing team support while also maintaining a high level of patient care and workload. RN Clinical Care Managers are experienced nurses who have displayed leadership, management, and communication skills. RN Clinical Care Manager performs nursing duties, ensuring policy adherence, attending meetings, developing services, acting as a patient services advocate, and collaborating with other professionals.
Job duties include, but are not limited to:
1. Utilizes nursing process to:
Follows the Nursing and Case Management process including assessment, planning, implementation, coordination and interaction through the referral process.
Coordinates and manages communication with patients, family and health care team to access health care services not available at OKCIC by utilizing referral resources (i.e. Indian Health Services, Medicare, Medicaid and other community resources).
Coordinates and manages communication with patients, family and health care team.
Ensures that services support quality patient communication, education, financial and tribal components.Records in EHR and RPMS accurately and timely. Responsible for processing referrals accurately and in a timely manner.
Communicates with a primary health care provider/health care team regarding patient referrals to assess outcome of referral for further treatment of the patient.
Provides patient education regarding the purpose of treatments, medications, disease process and any other necessary educational needs
Assesses patient needs to coordinate and manage patient care within the clinic, including triage and scheduling patients appointments.
Operates and ensures adequate supplies, medications and pharmaceutical supplies are available for patient treatment
Administers therapeutic measures as prescribed by the physician such as medications, wound and ear irrigations, etc.
Participates in assigned duties as designated by the Director and/or Clinical Case Manager to include, but not limited to participation on designated committees,
Responsible for the overall management of team approach nursing and coordination with other disciplines to provide quality care. Performs specialized skills as per clinic policies and procedures.
Maintains professional knowledge and proficiency through continuing education, in-services and workshops.
2. Direct employee and performance management
Directs employee performance management and provides ongoing education opportunities for staff members.
Responsible for ensuring compliance with Standards of Nursing Practice for the assigned department as well as local, state and federal, and regulatory agencies standards.
Implements, reviews, enters and revises all relevant Oklahoma City Indian Clinic and departmental policies and procedures.
Ensures compliance with the Clinics vision and mission and guidelines of health and regulatory agencies such as the Accreditation Association for Ambulatory Health Care (AAAHC) and the Centers for Disease Control and Prevention (CDC).
Leads the organization to and through best evidence practices.
Attends and participates in leadership, departmental and interdepartmental meetings (includes web and telephone as necessary).
Follows through on assigned action items to satisfactory completion.
Acts as a resource and provides education for patients, their family members and all health care professionals regarding Medicare, Medicaid, managed care and other payers.
3. Develops policies and procedures
Participates with the implementation of new policies and procedures.
Communicates policies and procedures for all OKCIC staff.
Promotes compliance with Nursing policies and procedures and makes recommendations as appropriate.
4. Population Healthcare
Implementing and ensuring the success of clinic processes leading to quality outcomes as related to defined population health initiatives.
Aim to improve the overall health of the communities we serve.
Volunteer in our community based programs.
Participates in identifying grants and/or developing grants to serve our community needs.
Develops and maintains knowledge and understanding of hospital and community resources, and facilitates use of the most appropriate level of care to conserve medical needs of patients.
5. Documentation and reporting
Reviews, documents and reports required elements of the variety of programs to the
appropriate entities.
Collaboration with other departments to obtain accrue charges, coding or other information needed for resolution of account.
6. Demonstrates Problem Solving and Conflict Management Skills
Identifies and analyzes problems or potential problematic situations and distinguishes between relevant and irrelevant information to construct logical solutions for the best possible outcome.
Communicates concerns to the Clinic leadership (Director and/or Clinical Case Manager) in a timely manner consistent with the magnitude of the situation.
Exercises good judgment by making timely, well-informed and effective decisions even when data is limited or solutions produce unpleasant consequences.
Manages and resolves conflict and disagreements in a positive and constructive manner, utilizing coaching when needed to achieve successful outcomes.
7. Performance improvement
Participates in monitoring and improving relevant outcome measures and/or quality improvement indicators.
Participates in improving clinic quality measures (GPRA) and other grant reporting responsibilities.

Graduate of an approved school of nursing. Bachelor of Science in Nursing preferred.
Current licensure as a Registered Nurse (RN) in the State of Oklahoma or current multistate license from a Nurse Licensure
Compact (eNLC) member state in good standing.
Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) issued by the American Heart Association are required in the first six months
Minimum three years of experience as a practicing professional nurse (RN), including experience in performance management and daily oversight of patient care.
Experience in tracking, trending, and analyzing clinical performance data preferred.
Participating in collaborating in the integration of regulatory requirements, accreditation standards, and guidelines.
Evaluating organizational compliance with AAAHC and OSHA standards when applicable.
