Title: U4Y Care Coordinator
Status: Full-time, nonexempt, in-person
Pay: $26 – $28 per hour
Schedule: Monday – Friday, 12:00pm – 8:30pm
Location(s): San Diego, CA (downtown)
Urban Street Angels, Inc. (“USA”) is a local non-profit organization founded in 2012 with a mission to end youth homelessness in the San Diego area. Currently, USA operates in three related areas. First, our supportive housing programs provide over 175 beds for transition-aged youth (“TAY” 18 – 24 years old) living on the streets in our community. Second, our rapid re-housing programs effectively bridge graduates of our programs to sustainable, long-term housing. Third, our 5-Tool Trade Academy provides access to vocational training, career pathways, and employment opportunities for TAY in our supportive housing and rapid re-housing programs.
United4Youth is a countywide initiative bringing together seven lead agencies and more than 40 youth-serving organizations and community partners to transform outcomes for current and former foster youth across San Diego County. Through coordinated services, shared resources, and strong partnerships, United4Youth connects young people ages 16–28 to opportunities that support housing stability, employment, mentorship, and long-term independence.
Position Overview: The Care Coordinator plays a vital role in the success of program participants by managing an assigned caseload of participants, ensuring they are successfully engaged in services, progressing through the program’s phased model, and maintaining housing stability. This role prioritizes coordination, accountability, and outcomes by connecting participants to specialized services and monitoring their progress toward education, employment, and permanent housing.
Job Duties:
- Serve as the primary point of contact for participants from intake through program exit
- Complete intakes, enrollments in HMIS/ETO and/or other internal systems, and initial assessments to identify needs and eligibility Develop and oversee individualized case plans aligned with the program’s phased model
- Connect participants to appropriate services, including Career Services Specialists (CSS), Occupational Therapy (OT), Housing Navigators, Behavioral Health, Medi-Cal, and community resources
- Manage a caseload of 45–50 participants
- Monitor participant engagement and progress across all phases, ensuring timely movement from stabilization to housing and intervene when barriers or disengagement occur
- Collaborate with CSS, OT staff, and Housing Navigators to ensure referrals are completed and services are effectively accessed
- Coordinate care across teams to support measurable progress toward education, employment, and housing outcomes
- Maintain responsibility for housing stability, including monitoring program compliance and addressing behavioral concerns using a trauma-informed and progressive approach
- Complete incident reports, documentation, and write-ups as needed
- Work to prevent terminations through early intervention and coordination
- Ensure accurate, timely documentation in HMIS and other systems, including case notes, service connections, and outcome tracking
- Participate in regular case conferencing and collaborate closely with staff to ensure youth are actively progressing through each phase of the program model and receiving coordinated support toward housing stability, income development, and independent living goals
Phase 1: Engagement & Stabilization:
- Complete intake, enrollment, and initial assessments
- Identify immediate needs and service eligibility
- Connect participants to CSS, OT, and behavioral health services
- Establish initial goals and case plan • Support program compliance
Phase 2: Active Participation & Skill Building
- Monitor engagement in education, employment, OT services, and/or mental health / substance use
- Track attendance, participation, and progress
- Address barriers to engagement and coordinate interventions
- Support income development and program compliance
Phase 3: Housing Navigation & Transition
- Refer participants to Housing Navigator
- Support program compliance • Support housing readiness and documentation
- Coordinate with housing staff to ensure smooth transition
- Prepare participant for successful program exit
Qualifications:
- Minimum of one (1) year of experience in TAY case management, care coordination, homeless services, behavioral health, social services, or a related field working with vulnerable populations. Minimum of one (1) year of experience working with TAY individuals experiencing homelessness, behavioral health conditions, substance use disorders, or system involvement preferred. Demonstrated knowledge of Housing First, trauma-informed care, harm reduction, and motivational interviewing principles
- Strong knowledge of local job resources, homelessness systems, public benefits, and community resources
- Ability to work effectively with individuals experiencing complex medical, behavioral health, and housing challenges
- Strong communication, organizational, problem-solving, and interpersonal skills
- Demonstrated ability to maintain confidentiality and manage sensitive information
- Ability to work independently and collaboratively within multidisciplinary teams
- Reliable transportation, valid California Driver’s License, and proof of automobile insurance
- Ability to lift up to 20 lbs and work in varied indoor/outdoor settings
- Must submit to a background check via Live Scan (with ongoing alerts), as required by specific programs, funding requirements and licensing regulations
- Must be authorized to work in the U.S.
- Negative TB test (within required timeframe)
Preferred Qualifications:
- Bachelor’s degree in Social Work, Human Services, Public Administration, Psychology, or a related field preferred
- Two (2) or more years of experience in employment navigation, homeless services, or case management
- Knowledge of San Diego County employment and community resources
- Experience supporting individuals with serious mental illness, substance use disorders, chronic medical conditions, or justice system involvement
- Experience assisting participants with Medi-Cal, CalFresh, CalWORKs, SSI/SSDI, or other public benefits.
- Training in Motivational Interviewing, Trauma-Informed Care, Harm Reduction, Crisis Intervention, De-Escalation, Suicide Prevention, or Domestic Violence
- Experience using electronic case management systems such as Efforts to Outcomes (ETO), HMIS, or related databases
- Bilingual in Spanish preferred
