Role Description The AFL Provider is a full-time, on-site role based in the Charlotte Metro area, responsible for delivering high-quality community and behavioral health services to participants. This role includes providing direct support and supervision in a family-like living setting, implementing individualized service plans, and promoting safety, personal growth, and emotional well-being.
The AFL Provider will coordinate daily activities, assist with life skills development, and ensure participants attend appointments, therapies, and community programs as needed. The position involves maintaining accurate documentation, collaborating with clinical and case management teams, and adhering to organizational and regulatory standards.
The AFL Provider will also communicate regularly with families, guardians, and other stakeholders to support continuity of care and positive outcomes.
Qualifications
- Candidates should possess strong interpersonal and communication skills, including the ability to build trust, listen actively, and interact effectively with youth, families, and interdisciplinary teams.
- Candidates should possess skills in behavioral health support and crisis management, such as de-escalation techniques, understanding of trauma-informed care, and experience implementing service or treatment plans.
- Candidates should possess organizational and documentation skills, including maintaining accurate records, following procedures, and managing schedules and appointments reliably.
- Candidates should possess mentoring and guidance skills, including the ability to model positive behavior, support life skills development, and encourage academic, emotional, and social growth.
- Candidates should possess cultural sensitivity and ethical practice, with a commitment to non-discriminatory, person-centered care and adherence to confidentiality and professional standards.
- High school diploma or equivalent required; an associate or bachelor’s degree in social work, psychology, human services, or a related field is preferred.
- Prior experience working with youth, individuals with behavioral health needs, or in community-based or residential settings is highly beneficial.
- Ability to work full time on-site in the Charlotte Metro area, with reliable transportation and flexibility to support participant needs.
- Relevant certifications (e.g., CPR/First Aid, CPI or other crisis intervention training) are a plus.
